the window.
To see a slide presentation on the Community Activities reporting
program
Click Here. A new window will open with a navigation frame
on the left hand side of the window. The slide you select in the left hand
frame will be displayed in the right hand frame. To return to this page, close
the window that opened up for the slide presentation.
To submit a Community Activities Report online, click on
one of the District links bedow. A report form will open in a new window. Fill
out the form. Those items marked with and asterisk (*) are required forms and
must be filled in. Submit only one project per report form
Verification of a project must be separately. You may email from
your regular email program to your District Community Activities Chairman by
clicking on the Verification links below or on the verification links on the
District form. Attach the verification (picture, receipt, letter of thanks,
etc.) as an attachment. When emailing the verivation, use the report
number as a submect line of the email.
Verification may also be sent by regular mail to your District
Community Activities Chairman
When you have completed filling out the form, click on the "Submit
Project" button at the bottom of the page. After the form has been transmitted
to your District Community Activities Chairman, you will be taken to another
page which will show you the information that has been transmitted. Print a
copy of this page for the Post records. At the bottom of this page, there is a
link that asks if you would like to "Submit Another Project?". Ingnore
it. If you want to submint another Project, close the current report window
and click on your District link again.
